Waiver of Liability & Photo Release

In consideration of being allowed to participate in any way in Chicago Inter Handball Club programs, related events

and activities including games, scrimmages, tournaments, etc., the undersigned acknowledges, appreciates and

willingly agrees that:

I will comply with the stated and customary terms and conditions for participation. If I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring the hazard to the attention of the nearest official immediately; and,
I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, traumatic brain injury or concussion, harassment or bullying, and severe social and economic losses which may result not only from their own actions, inactions or negligence but the action, inaction or negligence of others, the rules of play or the condition of the premises or of any equipment used. Further, I accept personal responsibility for the damages following such injury, permanent disability or negligent acts or omissions of others, and assume full responsibility for my participation; and,
I acknowledge that I am, to the best of my knowledge, of sound physical fitness to participate in handball play and,
I understand that I am required, by law, to carry health insurance and that any injury or losses incurred while participating in club activities are not covered by the club’s insurance policy.
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, hold harmless, Chicago Inter Handball Club, its officers, officials, coaches, team leaders, affiliated clubs, their advertisers, and, if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as “Releasees,” with respect to any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
I do hereby give my permission to Chicago Inter Handball Club leadership and/or event organizers to make decisions regarding my transport to receive medical attention in the case where I am unable to make these decisions.
I do hereby give Chicago Inter Handball Club, its assigns, licensees, and legal representatives the irrevocable right to use my name, picture, portrait, or photograph in all forms of media and in all manners, including composite, for finished product, including written copy, which may be created in connection therewith.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT; I FULLY

UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY

SIGNING IT AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
